Premier Care Management provides support and accommodation for vulnerable young people aged 16-25 years. We offer a safe and warm environment where young adults can gain valuable life skills training, good quality care and compassionate advice and support.

We're looking for Outreach Workers to work with our young people to help develop their values, skills, knowledge, self-control and direction to enable them to live independently.

This role is on 2 days on/4 days off rota basis and will include bank work. The shifts offered can vary in length and cover different times of the day and could be weekdays, weekends and Bank Holidays. You'll need to be available to do a reasonable number of shifts. We're looking for candidates who display good judgement and enthusiasm for working with young people who display very challenging behaviour. You'll also need to be a good team player.

Main Duties

  • To work to the requirements of Every Child Matters, Leaving Care Act 2018 and implement this within the outreach setting
  • To ensure that young people live in a safe environment where their welfare is paramount
  • To work within PCM's Policies and Procedures and the Company Statement
  • To engage effectively with PCM's young people and demonstrate a positive attitude towards them
  • To develop and guide our young people to develop the skills needed to live independently and manage a tenancy
  • To empower and educate young people to take control of their lives, particularly in training, education and employment
  • To help young people manage a budget and pay for utility bills
  • To help young people manage their accommodation and support them in keeping the home environment clean. This includes supporting with cleaning duties and gardening
  • To implement the care package that has been specifically identified for each young person
  • To be accountable for time spent with the young person and the actions taken
  • To complete daily contact notes following each shift completed
  • Where appropriate liaise with other domiciliary providers in regard to the young person's needs
  • To participate in delivering, reviewing and monitoring the young person's placement
  • To promote anti-discriminatory practices and equal opportunities in accordance with The Equality Act 2010
  • To attend regular team meetings, in-house training and supervision

Skills and experience

  • Excellent communication skills (both written and verbal) in order to record and communicate essential information
  • Knowledge of basic computer packages such as MS Word and Google Docs
  • An understanding of the demands of working with young people who may have complex needs and the ability to deal with these in a non-judgemental manner
  • The ability to work flexibly and have the availability to undertake shifts on evenings, weekends and bank holidays
  • QCF Level 2 or 3 in Health & Social Care (not essential)
  • Previous experience of working in a similar setting with young people who present with challenging behaviour
  • Experience working in a multi-racial environment, or working with people from other minority ethnic groups
  • Full UK driving licence and access to appropriate means of travel
